DENTISTRY OF ARIZONA MEMBERSHIP CLUB

The Dentistry of Arizona Membership Club (DAMC) is designed to provide greater access to quality dental care at an affordable price to those without dental insurance.

Unlike conventional insurance plans, with the DAMC there are no deductibles, no surprising same-day treatment restrictions, no yearly maximums, and no waiting periods to begin treatment. Membership club coverages begins immediately after plan registration.

ProPhylaxis Plan
Prophylaxis Plan

Ages 12 and younger

  • Semiannual exams + one emergency exam
  • 2 regular dental cleanings annually (excluding Full Mouth Debridement)
  • Routine diagnostic radiographs
  • 2 Fluoride treatments per year, no age restrictions
  • 20% savings on all Restorative treatment
  • 15% savings on all Cosmetic treatments (veneers, anterior crowns, Invisalign; excluding whitening)
Adult Membership
PERIODONTAL Plan

Ages 13 and older

  • Semiannual exams + one emergency exam
  • Routine diagnostic radiographs
  • Full Mouth Debridement
  • 20% savings on limited or full quadrant Scaling and Root Planning (SRP), irrigation, and laser therapy
  • 4 periodontal maintenance cleanings annually
  • 2 Fluoride treatments per year, no age restrictions
  • 20% savings on all Arestin therapy
  • 20% savings on all Restorative treatment
  • 15% savings on all Cosmetic treatments (veneers, anterior crowns, Invisalign; excluding whitening)

Our Membership Club Benefits include:

  • 2 Regular Cleanings or 4 Periodontal cleanings with laser gum therapy
  • All Exams (including yearly, limited, emergency and oral hygiene instructions)
  • Any Necessary X-Rays (including bitewings, panoramic and full-mouth)
  • 2 Fluoride treatments, no age restrictions
  • 15% Savings on Cosmetic Dental Procedures (including veneers, cosmetic anterior crowns, and Invisalign; excluding whitening)
If you currently pay $40 or more per month for yourself/spouse/dependents to receive dental benefits, our Membership Club may be a better financial alternative for you and your family.
